PCN Team

  • Dr Mathumai Kanthasamy is the Clinical Director Of Walthamstow Central PCN. She provides strategic and operational leadership to the PCN staff and member practices. Dr Kanthasamy also supports the development and implementation of strategic plans, leads on quality improvement initiatives and oversees workforce development across the network.
  • Satnam Bains is the PCN Manager of Walthamstow Central PCN, who oversees the operations and business infrastructure of the PCN, working alongside the Operational Lead, Clinical Director and member practices. She coordinates services across the PCN, manages the contractual requirements including PCN DES and Enhanced Service targets, and supports workforce planning and recruitment of ARRS roles.
  • Aqeeb Khan is the Operational Lead of Walthamstow Central PCN and oversees the day to day operational running of the services across the PCN, such as; Same Day Access, Enhanced Access, Phlebotomy Clinics, Larc Clinics, Wound Care services, etc.
  • Dr Janakan Crofton is the PCN Health Inequalities Lead for Walthamstow Central PCN and serves as the focal point and champion for addressing health inequalities within the network, coordinating activities/ projects and encouraging colleagues to embed equity considerations into all aspects of their work.
  • Dr Gabriel Ivibijaro is the PCN Multi-Disciplinary Team (MDT) Lead for Walthamstow Central PCN who provides leadership and line management for a team of practitioners working across member practices. He manages the coordination of the MDT, establishes and leads meetings across the PCN, and provide clinical leadership and mentorship to nursing and allied health professional colleagues.
  • Dr Munshi Zafar is the PCN Service Lead for Walthamstow Central PCN is responsible for leading and managing the clinical services within the PCN. He is responsible for driving improvements within the services, identifying challenges to effective service delivery, and implementing solutions.
  • Dr Abir Shaikh is the PCN Education Lead for Walthamstow Central PCN. He serves as an ambassador for education and training within the PCN, raising awareness and improving uptake of educational opportunities for the primary care workforce while championing training for all PCN staff.
  • Rhoda Akpan is an Advanced Clinical Nurse Practitioner within Walthamstow Central PCN. She has advanced skills and knowledge to allow her to expand her scope of practice to better meet the needs of the people she cares for.
  • Taha Munye and Thanam Ravagan are Clinical pharmacists working across Walthamstow Central PCN member practices. They are practitioners who clinically assess and treat patients using their expert medicines knowledge. They take responsibility for patients with chronic diseases and conduct structured medication reviews to proactively manage complex polypharmacy, particularly for elderly patients, care home residents, and those with multiple comorbidities.
  • Roisin Reilly and Sue Kofi are Social prescribing workers who are working across Walthamstow Central PCN member practices. They help people focus on what matters to them as identified in their care and support plan. They connect people to community groups and agencies for practical and emotional support.
  • Alexandra Kyriakidis is a Health and wellbeing coaches, working across Walthamstow Central PCN member practices. She uses health coaching skills to support people to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals. Alex supports people to self-identify and manage existing issues.
  • Ayoma Perera and Badhon Kashem are General practice assistants who work across Walthamstow Central PCN member practices to deliver a combination of routine administrative tasks and some basic clinical duties in the general practice setting. Their focus is on supporting GPs in their day-to-day management of patients, specifically aimed at reducing the administrative burden and making best use of consultations.
  • Ruby Leonard is a Mental health practitioner in Walthamstow Central PCN, who supports adults whose needs cannot be met by local talking therapies, but who may not need ongoing care from secondary mental health services. She acts as bridge between primary care and secondary mental health services and can facilitate onward referral to a range of services to meet patients’ needs.
